Application Form for MD/ MS Ayurveda

Important Instruction for Applying Online form

1. Before applying online or registering for application, candidates should have the following :-
A valid Email ID (If One do not have an email address we recommend creating a Gmail account).
2. A colour face close-up photograph taken within the last 3 months to reflect your current appearance.

3. Taken in front of a plain white or off-white background. Taken in full-face view directly facing the Camera.
4. Your photograph in digital form with Maximum Size : 30Kb, Image Dimension : 150W X 200H pixel resolution, and the Image should be in JPEG format.
Application Fees

BAMS – Rs.1,500/- (Rupees One Thousand Six Hundred Only).

MD/MS Ayurveda – Rs. 2,000/- (Rupees Two Thousand only).

Note: By using Net banking/ Debit card/ Credit card, If transaction of payment failed, you will receive SMS (due to network technical error). In this case, you need to pay once again after sometime to make the payment successful and after successful payment you may select the Center for Entrance Examination and click “submit” button for pdf form of application.

Eligibility Criteria

 For BAMS

1. Age Limit: Should complete the age of 17 years by 31 Dec 2016 and should not exceed 23 years by the said date.

2. Qualification: Must have passed the higher secondary examination or its equivalent in the first attempt with 60% marks in Physics, Chemistry & Biology taken aggregate, and English separately.

3. Those who appear for the qualifying examination in March / April 2016 can also apply.

 For MD / MS Ayurveda

1. A candidate who holds BAMS degree from a recognized University approved by CCIM, and has obtained full registration either from Central Council of Indian Medicine or any of the State AYUSH Council, after completing compulsory rotating internship.

2. Candidate must secure a rank in the Entrance test conducted by Amrita Vishwa Vidyapeetham.

3. Candidates who, after passing the final qualifying examination (BAMS), if undergoing twelve month compulsory rotating internship/practical training and is likely to complete the same on or before 31 July 2016.

4. Failure to produce the Registration certificate as above may lead to cancellation of Admission and in such case the candidate will be responsible to pay the Institution all the losses the institution may suffer on account of such cancellation.

5. Foreign candidates seeking admission from abroad shall submit their applications through the Ministry of Health & Family Welfare or Indian Council for Cultural Relations (I.C.C.R.) Govt. of India, New Delhi. A copy of the application form can also be sent to the Institute.

Important Instructions for Candidates appear in   Entrance Examination

1. Duration of Examination- 150 minutes.

2. Candidates will be allowed to enter the Examination Hall 30 minutes before commencement of the Examination.

3. Candidates will not be allowed to enter the Examination Hall after 30 minutes from the commencement of the Examination.

4. Candidates will not be permitted to leave the Examination Hall till the scheduled end of the Examination.

5. Candidates should leave answer sheet as well as the question booklet on the desk before leaving the Examination Hall. If the question booklet is not returned, the answer sheet will not be evaluated.

6. Candidates must bring their own Ball Point Pen, HB Pencil, Sharpener and Eraser.

7. Candidates are not allowed to bring any books, notes, mobile phone, paging device, slide rules, clerk table, calculator, laptop or any other electronic gadgets to the Examination Hall.

8. Candidates are not allowed to have any conversation or gesticulation or disturbance in the Exam Hall. Such acts of behavior could lead to the candidate being debarred and will be sent out of the Examination Hall.

9. During the Examination, the Invigilator will check the Hall Tickets to satisfy the identity of the candidate in his / her presence.

10. Candidates must make their own arrangements of stay for the purpose of Entrance Examination. If candidate lives far away from the Exam Center, he / she is advised to reach the city on the previous day.
